Pawsitive Energy Consent Form

I understand and agree that, while Pawsitive Energy LLC, will make every reasonable
effort to ensure my dog's/dogs' health and safety during his/her/their stay
with them, Pawsitive Energy, LLC cannot be held responsible for illness, injuries, or
loss resulting from normal interaction in a group or kennel situation. It is thoroughly
understood that I assume all risks relating to my dog's/dogs' stay with Pawsitive Energy, LLC.

In the event my dog/s must be taken to a veterinarian for treatment of illness or injury
during his/her/their stay, I will be responsible for any and all veterinary fees up to the
amount I have preapproved for emergency veterinary care. I understand that
Pawsitive Energy, LLC will generally use either a local veterinarian or MedVet for treatment.

I am requesting that the veterinarian contacted by Pawsitive Energy, LLC
do whatever is deemed medically necessary in administering treatment/s or caring for my dog/s,
whether treatment is medical or surgical, up to my preapproved emergency amount.

I will take responsibility for making the decision for treatment beyond the
preapproved emergency amount. If I have not provided a preapproved emergency amount
for care, and I am unable to be contacted in a timely manner, I request that Lori Price,
owner of Pawsitive Energy, LLC, use her best judgment regarding my dog's/dogs'
care, and that I will be responsible for any and all charges relating to that care.

I further agree that I will not hold Lori Price, Pawsitive Energy, LLC,
and/or any attending veterinarian in any way responsible or liable for any decision
that may be made in such a situation.

I acknowledge that the treating veterinarian will use all reasonable precaution
against injury to, escape or death of my dog/s, I agree that I will not hold him/her liable or
responsible in any manner whatsoever and under any circumstances relating to the care,
treatment or safekeeping of my dog/s, or otherwise in connection therewith, as it is thoroughly
understood that I assume all risks associated with such medical treatment.

I also understand and agree to comply with Pawsitive Energy, LLC's
policy regarding abandoned animals.

I understand that failure to accept these terms will result in a cancellation of my
dog's/dogs' Pawsitive Energy, LLC, reservation.

This form shall remain in effect for 12 months from the date of signing.

Abandoned Dogs

I understand that the Pawsitive Energy, LLC policy
regarding abandoned animals is as follows:
If I leave my dog/s for 5 or more days beyond what
has been scheduled and do not contact Pawsitive Energy, LLC
to arrange extending the dog's/dogs' stay, Pawsitive Energy, LLC,
will make one attempt to contact me through
each of the phone numbers and email addresses I have provided.

If, after 10 days of the scheduled pick up date,
Pawsitive Energy, LLC and I have not been in contact,
and I have not received written approval from Pawsitive Energy, LLC
extending my dog's/dogs' approved stay, my dog/s will automatically
become a resident/residents of the Pawsitive Energy, LLC sanctuary.

Pawsitive Energy, LLC will determine what is in the dog's/dogs'
best interest regarding his/her/their future home,
and I will have no further claim to him/her/them.

I will still be responsible for payment of the boarding fees for the
originally scheduled visit, and agree to pay via cash or certified check.
